I authorize Unified Array LLC (hereafter "UA") to provide managed IT services, technical support, network infrastructure work, device management, remote access administration, VoIP configuration, and/or any other technology services (collectively, "Services") to my organization and its associated systems, devices, and infrastructure. I understand that UA personnel are trained professionals, but that UA is not an authorized service dealer for any specific manufacturer. I agree to release, indemnify, and hold harmless UA from liability for any claims or damages of any kind that may arise from Services performed, even if caused by negligence of UA or its agents. I understand that UA is not responsible for data loss that may occur as a result of Services rendered.

Acknowledgements — Check Each Box

Authority & Authorization I certify that I am authorized to engage UA to perform Services on the systems, devices, and infrastructure¹ belonging to or managed by my organization. In the event that I am found not to have such authority, I assume all personal liability for any claims arising from Services rendered, including those asserting negligence on the part of UA. I further understand that Services rendered — including firmware updates, OS upgrades, configuration changes, hardware replacements, or third-party software installations — may void manufacturer warranties on affected devices, and that UA does not assume any liability or warranty obligation in such cases.

Data, Liability & Remote Access I expressly waive all claims against UA for any loss, corruption, or unavailability of data² incidental to Services rendered, including data stored on servers, workstations, NAS devices, cloud environments, or any network-connected device. UA's liability is strictly limited to damages directly caused by gross negligence while Services are actively being performed. I further acknowledge that remote monitoring agents or management software installed by UA are the property of UA and/or its licensed vendors, and I agree to cooperate with their orderly removal upon termination of any service agreement.

No Warranty & Payment I understand that UA offers no verbal or written warranty, expressed or implied, regarding the outcome or continued performance of any system, device, network, or service following completion of work. I certify that I will pay, in full, for all Services rendered at UA's current rates as agreed upon in the applicable service agreement or statement of work, and that non-payment may result in suspension of monitoring, remote access, and support services.

1. Systems, devices, and infrastructure is defined as: all hardware (servers, workstations, laptops, networking equipment, switches, routers, firewalls, access points, printers, monitors, and associated cabling), software (operating systems, applications, firmware, and configurations), VoIP systems, cloud-hosted resources, and any other technology assets owned by or operated on behalf of the client organization.

2. Data is defined as: any information, files, databases, configurations, or records stored on or transmitted through any system, device, server, cloud environment, or network-connected resource belonging to or managed on behalf of the client organization — regardless of format or storage medium.
